The Sports Injury Clinic’s Type 2 Diabetes Program is a Medicare initiative which enables people who have type 2 diabetes access to 9 services (one initial and 8 groups) with an exercise physiologist per calendar year. These services are in addition to the five individual services available under the enhanced primary care program (if eligible).

The Sports Injury Clinic’s Type 2 Diabetes program includes a one on one initial assessment with an exercise physiologist to prepare you for the group sessions. The assessment involves looking at your diabetes and physical activity history, identifying any individual goals that you may have and allows us to provide you with the individualized care you need within the group sessions. The group sessions are comprise of an exercise component and an education component looking at different issues relating to type 2 diabetes such as nutrition, exercise and hydration and how to safely exercise at home.
To be eligible for the program you must have Type 2 diabetes and be referred by your GP under a management plan.
